Adverse pregnancy outcomes (APOs) are associated with poor outcomes for mother/infant dyads, as well as future cardiovascular risks for the mother. Significant racial disparities exist and are deeply rooted in the legacy of racism. Our solution is to build up trust by pairing Black mother/infant dyads who experienced an APO with a community health worker (CHW) for one year after childbirth. CHWs will serve as a bridge to health for mothers and babies.
